Personal Information: This information will be collected when you are signing up on our client portal.

– Your social security number.

– Your spouse’s full name, date of birth & social security number or tax ID number

– Identity Protection Pin (If Applicable)

– Account & Routing number to receive refund by direct deposit OR pay your balance if you choose to (unless you prefer a check mailed)

– Prior year’s tax return (Recommended but not required)

Dependent(s) Information: This information will be collected when you are signing up on client portal.

– Dependents full name, date of birth social security & number or tax ID number.

Sourced Of Income: Only a select few of these forms are necessary for your annual tax filing based on your individual circumstance. 

Employed

– Form W-2 (Wages From Employer)

– Form 1099-NEC (Non Employee Compensation)

– Form 1099-K (3rd Party Transaction Payments)

Unemployed

– Form 1099-G (Unemployment Compensation)

Retirement Income

– Form 1099-R (Pension/IRA Income)

– Form SSA-1099 (Social Securtiy Income)

Savings, Interest & Dividends

– Form 1099-INT (Interest Income)

– Form 1099-DIV (Dividends & Distributions Income)

– Form 1099-B (Proceeds From Broker & Barter Exchange Transactions)

– Form 1099-S (Proceeds Of Rental Estate Transaction)

Other Income/Documents

– Form W2-G (Gambling)

– Form 1099-MISC (Royalty & Other incomes paid)

– Record of alimony received/paid with ex-spouses name and social security number.

Deductions: More than likely you won’t need all the documents below. Your life situation determines the types of deduction you can take.

Education Expenses

– Form 1098-T (Tuition Statement)

– Form 1098-E (Student Loan Interest)

Childcare Expenses

– Record/Receipts of childcare with provider including EIN – Child must be under 13

 K-12 Educator Expenses (Teacher)

– Records/Receipts Of Classroom Out Of Pocket Expenses

Retirement/Savings

– Form 5498 (IRA Contributions)

– Form 5498-SA (HSA Contributions)

Medical Expenses

Unreimbursed amounts paid to doctors, dentists, hospitals, insurance, and healthcare

Unreimbursed amounts for qualified insurance outside of an employer provider plan or marketplace

Vehicle & Home Ownership

– Electric Vehicle Information

– Records/Receipts For Energy Saving Home Improvements (Ex: Solar panels)

– Records/Receipts of personal property & real estate tax

– Form 1098: Mortgage Interest Statement

Charitable Contributions

– Record/Receipts of cash amounts donated to a charitable organization

– Records/Receipts of non-cash charitable donations

– Number of miles driven for charitable purpose

Health Insurance

– Form 1095-A (If you are enrolled in an insurance place through the marketplace)

State & Local Taxes

– Amount of sales tax or state and local income tax paid

– Record/Receipt of vehicle sales tax paid
